What is included in my annual membership? How much does it cost?

What are my responsibilities as a member?

• Entire membership fee must be paid up front and is valid for 12 months
• Membership fee is non-cancellable and non-refundable
• To receive discounts on treatment, treatment fee must be paid in full on the day of service, no exceptions.
• Failure to show or cancellation of a scheduled dental cleaning appointment without the required 48-hour notice will count as one of your dental cleaning occurrences and cannot be made up.
