Written estimates will be provided for all recommended treatment and for all cosmetic choices.

Payment IN FULL is required on the date services are rendered. Our patients can pay for their office visits and any treatments by cash, personal bank card (debit card), credit card or check.

If necessary and applicable for your diagnosed situation, we can plan your treatment over time to assist in the out-of-pocket expenses. However, please know that delaying treatment may advance conditions and require additional treatment expenses. This option should be discussed in detail with Dr. Sullivan.



We will gladly process your insurance claim, however, we do not participate with insurance companies. Please bring the necessary information with you on your first visit.

Insurance policies generally cover only a portion of the total treatment cost. Unless other arrangements have been made, we ask that you pay your portion of the bill at the time of treatment. It is your responsibility to pay any balance not paid by your insurance company.

For your convenience, we also accept payments made with your Visa, MasterCard, Discover, and American Express. Financing is available through Care Credit.
